SBBL Liability Waiver

SKYBALL BASKETBALL LEAGUE

TRYOUT WAIVER, RELEASE OF LIABILITY, ASSUMPTION OF RISK & MEDIA RELEASE

​

ACKNOWLEDGMENT OF RISK

I understand that participation in SkyBall Basketball League tryouts, practices, demonstrations, exhibitions, competitions, and related activities involves inherent risks, including but not limited to:

  • Falls and collisions

  • Sprains, strains, fractures, and broken bones

  • Head, neck, back, and spinal injuries

  • Concussions and traumatic brain injuries

  • Cardiac events

  • Permanent disability

  • Death

  • Injuries resulting from equipment, playing surfaces, trampolines, mats, basketball goals, facilities, spectators, participants, or staff

I understand that SkyBall is a unique basketball league played on specialized surfaces that may increase certain risks associated with jumping, landing, and player contact.

​

I voluntarily assume all risks associated with participation.

​

RELEASE OF LIABILITY

In consideration for being permitted to participate in SkyBall Basketball League activities, I hereby release, waive, discharge, and covenant not to sue:

SkyBall Basketball League
SkyBall Sports & Entertainment
VoiceItRadio Corp.

SkyZone


Its owners, officers, directors, employees, volunteers, contractors, sponsors, affiliates, agents, representatives, event staff, and venue partners

Including but not limited to:

  • Sky Zone

  • Property owners

  • Facility operators

  • Event sponsors

  • Media partners

from any and all claims, liabilities, demands, actions, causes of action, damages, costs, expenses, attorney fees, injuries, losses, or death arising from or related to my participation.

This release applies regardless of whether the injury, loss, or damage results from negligence, equipment failure, participant conduct, facility conditions, or any other cause, to the fullest extent permitted by law.

MEDICAL AUTHORIZATION

I certify that I am physically capable of participating in strenuous athletic activities.

I understand that SkyBall does not provide medical insurance.

I am responsible for all medical expenses arising from my participation.

In the event of an emergency, I authorize SkyBall representatives to obtain emergency medical treatment on my behalf.

I accept full responsibility for any resulting medical expenses.

NO GUARANTEE OF SELECTION OR COMPENSATION

I understand and acknowledge that:

  • Payment of a tryout fee does not guarantee selection.

  • Participation does not guarantee a roster spot.

  • Participation does not guarantee employment.

  • Participation does not guarantee compensation.

  • Participation does not guarantee sponsorship opportunities.

  • Participation does not guarantee future participation in the league.

All roster decisions are final.

MEDIA RELEASE

I grant SkyBall Basketball League, VoiceItRadio Corp., sponsors, partners, broadcasters, and media affiliates the unrestricted right to photograph, record, livestream, broadcast, reproduce, edit, distribute, and use my name, image, likeness, voice, and performance for promotional, advertising, marketing, broadcast, social media, website, sponsorship, television, streaming, and commercial purposes.

I understand that I will receive no additional compensation for such use unless otherwise agreed to in writing.

INDEMNIFICATION

I agree to indemnify, defend, and hold harmless SkyBall Basketball League, VoiceItRadio Corp., Sky Zone, sponsors, venue partners, and their respective owners, officers, directors, employees, volunteers, and agents from any claims, lawsuits, damages, liabilities, costs, or attorney fees arising out of my participation in SkyBall activities.

GOVERNING LAW

This agreement shall be governed by the laws of the State of Ohio.

If any portion of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

​

PARTICIPANT CERTIFICATION

I have carefully read this Waiver and Release of Liability.

I understand its contents.

I understand that I am giving up substantial legal rights by signing this document.

I sign this agreement voluntarily.

Note: Participation is limited to individuals 18 years of age and older. This waiver should be reviewed by an Ohio attorney before final implementation to ensure maximum enforceability and compliance with Ohio law.
